IMAGINE PERFORMING ARTS CENTER PARTICIPATION AGREEMENT, LIABILITY WAIVER, AND POLICY ACKNOWLEDGEMENT

IMAGINE PERFORMING ARTS CENTER PARTICIPATION AGREEMENT, LIABILITY WAIVER, AND POLICY ACKNOWLEDGEMENT. PLEASE READ CAREFULLY. BY CHECKING THE ACCEPTANCE BOX AND SIGNING ON THE REGISTRATION PAGE, YOU ACKNOWLEDGE AND AGREE TO ALL TERMS AND CONDITIONS OUTLINED HEREIN.

PAYMENT POLICY: I understand that Imagine Performing Arts Center operates on a 10-month class session, with annual tuition divided into equal monthly payments. Monthly tuition is due during the first week of each month, regardless of the number of class weeks within that month. Imagine Performing Arts Center reserves the right to adjust payment schedules as needed and may assess late fees on unpaid balances.

ADVERTISING & PROMOTION CONSENT: I understand and agree that any photographs, video recordings, audio recordings, or film footage captured during classes, activities, or events hosted by Imagine Performing Arts Center (both on- and off-site) become the property of Imagine Performing Arts Center. I grant permission for Imagine Performing Arts Center to use, reproduce, or distribute these images and recordings for promotional, marketing, and publicity purposes on social media, the website, and other media channels. I understand that I may contact Imagine Performing Arts Center directly if I have concerns or would like to discuss media usage limitations.

CONSENT TO MEDICAL TREATMENT AND INSURANCE REQUIREMENT: I acknowledge that Imagine Performing Arts Center does not carry individual medical insurance for students or participants. I certify that I and/or my child are covered by our own family or personal insurance policy, which shall serve as the primary source of coverage for any injury or illness incurred while participating in activities at or in association with Imagine Performing Arts Center. I further understand that Imagine Performing Arts Center shall not be responsible for providing or reimbursing any medical treatment or expenses related to injuries sustained while participating in any class or event.

MEDICAL INFORMATION DISCLOSURE: I understand that disclosing relevant medical information on this form is optional. However, I acknowledge that Imagine Performing Arts Center cannot be held responsible for any consequences arising from a lack of disclosure regarding my or my child’s medical history or any medical conditions that may impact participation.

LIABILITY WAIVER AND ASSUMPTION OF RISK: I acknowledge and accept that participation in classes, training, performances, and other events at Imagine Performing Arts Center involves inherent risks, including but not limited to bodily injury, illness, and property damage. On behalf of myself, my child, and/or any other individuals for whom I am legally responsible, I voluntarily assume full responsibility for these risks and waive any and all claims against Imagine Performing Arts Center, its directors, employees, instructors, volunteers, and agents, arising from injuries or damages incurred as a result of participation in any activities, whether occurring on the premises or off-site.

I further understand that dance and barre instruction involve kinetic corrections that may include physical contact by the instructor to correct alignment and posture. If I or my child have any concerns about physical contact, I understand that it is my responsibility to address these concerns with the instructor prior to participating in any class.

CLASS CANCELLATION AND REFUND POLICY: I acknowledge and agree that Imagine Performing Arts Center does not provide refunds for missed classes, including those canceled due to inclement weather or other unforeseen circumstances. While efforts will be made to provide make-up options for missed classes, I understand that availability is not guaranteed. In cases where a student must miss an extended period of time due to medical conditions, I understand that I may contact Miss Sarah to discuss alternative arrangements on a case-by-case basis.

POLICY ACCEPTANCE AND ACKNOWLEDGEMENT: I have carefully read, understood, and agreed to abide by all policies and procedures outlined by Imagine Performing Arts Center, including but not limited to the payment, advertising, medical consent, and liability waiver sections above. I understand that my participation in Imagine Performing Arts Center’s programs is contingent upon compliance with these terms and that this waiver applies to all current and future activities in which I and/or my child may participate.
